We have considerable experience in this area, working in multi-disciplinary teams and case managers throughout the Midlands, Lincolnshire, Yorkshire, and North of England.

We are happy to carry out home assessments and can provide ongoing support in the home environment or virtually depending on the patient’s preferences. Alternatively, we can see clients at our purpose-built premises.
